How does CBT work? From the first appointment to practice between sessions

Cognitive behavioural therapy is a psychological therapy planned with a trained professional around your difficulties and goals. There is no single script for a CBT session. The process can include assessment, shared goals, exploring patterns of thinking and behaviour, and trying suitable skills in everyday life.

What happens at the first appointment?

The practitioner asks about the difficulties bringing you to therapy, their effect on your life and what you would like to change. Health, past experiences and existing support may also matter. Assessment is an opportunity to consider whether this approach suits you and how to work safely. You can ask questions too.

Building an understanding together

You may explore links between a situation, thoughts, emotions, physical responses and actions. For example, avoiding speaking in a meeting might bring short-term relief while making the next meeting feel harder. This is a fictional illustration, not an explanation of everyone’s avoidance. Goals should connect with your own circumstances and needs.

What happens during and between sessions?

A plan might include thought records, problem solving or suitable behavioural work. Methods vary with the difficulty; CBT is not the same collection of exercises for everyone. Practice between appointments can be discussed at the next session. An unfinished task offers information about what made it difficult. You can discuss work you do not feel ready to attempt.

How long does it take, and where does an app fit?

Length and frequency vary with needs, treatment approach and service. This page cannot determine a personal session count. Progress and the plan are reviewed together. A thought diary app can make recording easier, but does not provide the complete assessment and therapy process. Before starting, ask about a practitioner’s training, approach, fees and confidentiality.
